We are reducing maternal and infant mortality in vulnerable groups by working within our specialty area to optimize healthcare, starting at birth. Our multidisciplinary team works collaboratively to center mothers and babies in care, and meet their needs in the first weeks of life. Our team consists of advanced practice nurses, lactation consultants, and doctors who are passionate about improving outcomes by increasing access to healthcare. Our clinics provide comprehensive primary care for newborn babies, extensive health screening and referral services for mothers, and inclusive feeding support. We are proud to provide care to all families who seek our services, regardless of their parenting decisions, family structure, or ability to pay. Co-ordinated referral processes help to prevent fragmentation of care by taking advantage of a warm hand-off. Research and common sense tell us that moms and babies win when providers communicate effectively across the continuum of care.

Benefits of opening the door for communication between the patient’s medical home and our specialty practice include:

  • Ensures the patient gets the appropriate care and the issues you referred for are addressed while minimizing confusion and gaps in care.

  • Providing a direct referral impresses upon the patient the significance of the provider’s recommendation and increases the likelihood that the patient gets the advised care.

  • Elevates your practice’s image to the patient and enhances the patient-provider relationship.

  • Avoids self-referrals. Despite their best efforts, patients cannot – and should not be expected to – fully succeed in coordinating their care.
